Do and Don't for Sport Parents

Reimbursement Sheet
Should be used in requesting reimbursement. Contact Mark Gassaway if you have any questions. Please note the following:

1. All expenses have to be submitted no later than July 31.
2. Check NBC By-Laws for applicable reimbursement expenses.
3. Receipts are required (originals for NBC tax return purposes).
4. Please put in Mark Gassaway's file. Include your telephone number so we can let you know when your check is available.

Mileage to Gymnastics Meets

USAG Night

Please follow the directions below and return all necessary documents on USAG night.

1.  Please read the above links to the Booster Club By-Laws and Budget & Financial Procedures.  Once you have read the by-laws and budget documents then print the form here, sign and return.  Please note that on this form the parent or guardian is the active member of the booster club.

2.  Please read the Parent Do and Don't information posted above.

3.  Please print and sign the media release form.  This form gives the Naydenov Booster Club permission for things like using pictures for the website, to print the gymnasts scores in the newspaper, and any other media related topics. Media Release Form

4.  Please print and sign the Individual Webpage Release Form.  Regardless if you want your child to have her own page please sign and return this form.  By signing you give permission to create a webpage link through her team page to view her bio, scores, and picture.  However, only the information you give will be placed on the website.  For example, you can opt not to submit a photo.  Scores will all be automatically posted after each meet unless you request that you otherwise don't want them to.

5.  If you choose to do an individual webpage please fill out the form here to place any bio information you want posted and you can email a picture to jcllml06@hotmail.com

6.  Please Print and return the USAG number registration form.  Please only fill in your gymnast info, we will fill in club number, etc.
